Action Medicine Consultants, LLC (BOC AP#: P12095) is approved by the Board of Certification, Inc. to provide continuing education to Athletic Trainers (ATs). This program is eligible for a maximum of 7.75 Category A hours/CEUs. ATs should claim only those hours actually spent in the educational program.

***HOW TO OBTAIN YOUR CME CREDITS ***
All event attendees must have a CloudCME account in order to record attendance and/or claim credit. Physicians must also set up their profiles correctly in order for credits to be accurately reported to state and specialty boards. We highly recommend that this is completed prior to attending the activity. Instructions for creating an account, setting up your profile, and downloading the CloudCME mobile app are attached. Please share these resources with your activity’s frequent attendees and remind them to sign up and complete their profile!
ATTENDANCE CAN ONLY BE RECORDED 60 MINUTES PRIOR TO THE ACTIVITY START TIME, DURING THE ACTIVITY, AND UP TO 120 MINUTES AFTER THE ACTIVITY CLOSES.
Activity Evaluations and Certificates: After the attendee completes their chosen method to record their attendance/claim credit, an evaluation for the activity will be applied to their CloudCME account. Certificates of attendance will be available upon completion of the activity evaluation. Credits will be awarded and reflected on the attendee’s transcript upon completion of the evaluation. From I-110 @ Acadian Thruway Exit (East or West Bound)
Exit at South Acadian Thruway (Exit 157B).
If exiting from the eastbound off-ramp, turn right onto South Acadian Thruway.
If exiting from the westbound off-ramp, turn left onto South Acadian Thruway.
Remain straight on South Acadian Thruway past Perkins Rd. (turns into Stanford Avenue). Stay in the right-hand lane past Morning Glory Ave. and Hyacinth Ave. Once you approach the University lakes (on your right) Continue Straight at the light on LSU Avenue.
At the next light (Highland Road) turn right.
Go past the next light and will veer left onto S. Quad Drive.
At next light you will veer right onto Nicholson Road.
Go to next light and turn right onto South Stadium Drive (LSU Tiger Stadium is on your right)
Go to the first entrance before the Security gate, turn right, and park in South West Stadium Lot #401 (commuter / X designated lot)
SOUTH STADIUM CLUB (Located at Gate 20 in South Stadium) - after parking proceed to the RIGHT - you will see the plaza area.
Go to level "CL" in South Stadium Club elevators.

Cancellation/Refund Policy
Course Cancellations: Action Medicine Consultants, LLC and the LSU OLOL Championship Healthcare Partnership reserves the right to cancel any course for various circumstances, including if enrollment is not sufficient to warrant offering of the course. If a course must be canceled for any reason, a full refund via check or full refund to credit debit card used to purchase seat(s) will be granted to reflect the participant's original payment. If payment was made by an organization for a single or multiple registrants, the organization will be refunded the registration fees.
Refund Policy: Full refunds (Minus 10% for handling fees) are given after completion of the course if cancellation request is received 14 days prior to the seminar date. No refunds will be given after this deadline, but fees may be used to attend a future course.
